REMINDER TO FILL OUT BENEFICIARY FORMS

February 14, 2025

All FFAW-Unifor members in good standing under the age of 70 are included in the Union’s Life and Accidental Death and Dismemberment Insurance policy, but an incomplete beneficiary form can leave your loved ones with the burden of navigating long legal delays.

Valued at up to $15,000, the policy gives you the peace of mind knowing your loved ones have added protection and security in case the unexpected ever happens.

IMPORTANT: ORIGINAL Beneficiary Forms MUST be returned either by mail or in-person. Scanned, faxed or emailed copies will not be accepted by the Insurance Companies.
