FFAW members are reminded that the Health Benefits Plan and Dental Plan are two separate offers, with separate enrolment processes, providers, deadlines, and contact information.

Members interested in both health and dental coverage are encouraged to review the information for each plan and complete both enrolment processes before the applicable deadlines.

👩‍⚕️HEALTH BENEFITS:

The discounted health benefits plan from Belmont Health is available to FFAW members for an annual payment of $2,504.70 for one year of coverage.

⚠️The enrolment deadline for this plan is June 30, 2026.

Enrolling is easy and can be done right from your phone:
Step 1: Print off the Belmont Health enrolment form.
Step 2: Complete both pages, making sure to sign and date both pages.
Step 3: Take a clear picture of both completed pages.
Step 4: Email the pictures directly from your phone to FFAW@gobelmont.ca.
Step 5: Send EMT information as per the instructions on the form.

🦷DENTAL PLAN:

Due to the number of inquiries about dental coverage, Cal LeGrow has secured a separate offer for FFAW members through Medavie Blue Cross.

Enrolment is open from May 22 to July 15, 2026. Applications submitted during this period will receive immediate coverage effective July 1, 2026, with no waiting period.

Plan highlights include:
• No waiting period
• No annual maximum
• 70% coverage
• One cleaning per year
• Coverage for fillings, root canals, extractions, and more
• AD&D coverage for members and eligible dependents
• No medical underwriting required

Members who enrol after July 15, 2026, will be subject to a six-month waiting period.
