URGENT NOTICE – Health Plan Enrolment Extended

Due to the Canada Day holiday this week, and because many of our members are still on the water fishing, we have successfully secured a two-week extension for Health Plan enrolment.
The new and absolute final deadline to enrol is Friday, July 17. No enrolment forms will be accepted after this date.
This extension is a final opportunity for members who have been unable to sign up because of work commitments or the holiday. If you know members who have not yet enrolled, please encourage them to take advantage of this extension and spread the word.
Enrolment is simple:
Application Process:
- Step 1 – print off application
- Step 2 – complete both pages with signature and date on both pages
- Step 3 – take a picture of both pages
- Step 4 – email form from phone directly to email address on enrolment form (FFAW@gobelmont.ca)
- Step 5 – send EMT info as per instructions on form
